FAA's MedXPress

Before your appointment you must do the following:

  • Complete your medical application with the FAA
  • Bring your confirmation # to your appointment so that I may access your medical application

Please also bring:
  • A Picture ID
  • A printed copy of your completed application (FAA Form 8500-8)
  • Any prior FAA documentation (Special Issuance [SI], Statement of Demonstrated Ability [SODA]) and any associated documents outlined within
  • Supporting medical documentation for diagnosed medical conditions (statements from your physician attesting to your medical well-being for your condition, a log of your blood pressures for hypertension, a list of current medications, etc.)
  • For Medications: Know the names, dosage, and schedule of any medications you take routinely
  • For Vision: Bring any corrective lenses that you use (may use) when flying
  • A urine sample is required at the visit.  It is not a drug screen.  Sugar and protein are checked to evaluate general kidney function and to assess for early diabetes.

Please be aware that once the electronic medical application is submitted it can not be changed. You are able to save the form and return to it at a later date prior to submitting.  If you have any questions, please call our office prior to submitting your application.
 
If you have had any significant change in your medical condition (are on new medications, have a new diagnosis, had major surgery) or have a previously unreported DUI or alcohol-related offense, additional clinical information may be required.  



If you have any questions or concerns about passing your flight physical, come in for a consultation and review before filling out MedXPress or coming for your flight physical.
